Dear Ministers and Elected Government Officials,
I am writing to you on behalf of my child, (NAME), (AGE). (NAME) was diagnosed with (DIAGNOSIS) in (YEAR). (INSERT THERAPIES HERE) were recommended in order for my child to access education and reach their potential. (NAME) has not received intervention since (YEAR).
Without access to the recommended therapies, (NAME) struggles with (CHOOSE/ADD HERE: Activities of daily living, sensory dysregulation, emotional regulation, mental health, school refusal, self injurious behaviours, communication, food aversions, pain, mobility issues, safety awareness, play, sleep etc).
Access to (INSERT THERAPIES HERE) would enable my child to ( INSERT/ADD HERE: participate in education, contribute to ADL's, take part in play and events, feel more emotionally stable, reduce financial strain, lower our family's stress levels etc..)
Please consider this correspondence as an official complaint regarding the abysmal and inadequate support (NAME) has had access to under the HSE and associated organisations. (I/WE) are prepared to move forward with a complaint to the Ombudsman for Children.
(I/WE) refuse to accept this discriminatory treatment any longer. (I/WE) look forward to your response.
